Evaluation of and fitting for adaptive equipment
Instruction in energy conservation, work simplification and joint protection
Perceptual motor training
Intervention for psychiatric disorders
Adaptive training for successful completion of daily living activities
Treatment of speech, hearing and language problems.
Treatment is Designed to Improve any and all of the Following
Problem solving and memory
Assistance for bathing, grooming and personal needs of the patient.
Assessment of financial and environmental needs of the patient.
Referral to social service agencies and assistance programs.
Other Components of the Social Worker's Role Include
Liaison with other members of the health care team
Advocacy with community agencies
Information and referral to programs
Counseling and crisis intervention, application for financial assistance
Assistance with long-term care planning, placement counceling
Home Care Unlimited of El Paso will furnish services to all patients utilizing a plan of care developed by the agency staff in consultation with, under the supervision of and approved by the patient's physician.
